The paperwork
We do the administrative legwork
Records do not move themselves between clinics, and you should not have to project-manage your own transfer. Here is who does what.

Send what you have
A treatment summary, recent letters, or your own notes all help your Clinician prepare. If you do not have everything to hand, you can still book, and records can follow.

We request your records
With your written consent, our patient-support team requests a copy of your records and treatment summary from your previous clinic, so you are not chasing paperwork yourself.

Your GP stays in the loop
If you would like, your Clinician can write to your regular GP after your consultation, so the people involved in your care are working from the same picture.
How it works
Moving your care, step by step

Start with a call, or check your eligibility
Book a free 10-minute info call with our patient-support team to ask how the clinic works, or complete the online eligibility check in your own time. No clinical advice is given on the call.

Share your history and give consent
Complete your intake forms and tell us where you have been receiving care. With your consent, our team requests your records from your previous clinic. Anything you can share directly helps your Clinician prepare.

A full consultation
You meet your Clinician by video. They review your history and records with you, discuss your circumstances, and make an independent clinical assessment. Nothing carries over automatically. That is deliberate, and it protects you.

Ongoing care with the same Clinician
Your follow-up appointments are booked with the same Clinician, and reviews are scheduled rather than left to chance. With your consent, your Clinician can write to your regular GP so your wider care stays joined up.
Continuity
One Clinician, the whole way through
The Clinician who takes your first consultation is the Clinician you see at your reviews. They know what has been tried, what changed, and what you said last time, because they were there. You should not have to start your story from scratch at every appointment.

Consultation fees are published in full, including the cancellation policy. There are no membership or subscription fees.

Changing clinics: common questions
Can I move my care to LeafLine Clinic?
Many people come to us from another telehealth clinic. The first step is a consultation with one of our Clinicians, who reviews your history and discusses the options available to you. You are welcome to start with a free 10-minute info call if you would like to understand the process first.
Can LeafLine request my records from my previous clinic?
Yes. With your written consent, our patient-support team can request a copy of your records and a treatment summary on your behalf. You can also ask your previous clinic directly. Australian privacy law gives you the right to a copy of your own health records.
What should I have ready?
A summary of your current care and any records your previous clinic can provide are helpful for your Clinician to review. If you do not have everything to hand, that is fine. You can still book and bring what you have.
Will my care continue exactly as it was?
Your LeafLine Clinician makes an independent clinical assessment at your first consultation. They review your history and records, discuss what has and has not been working, and decide with you what is appropriate from here. Nothing is carried over automatically.
What does it cost to move my care?
LeafLine does not charge a fee for receiving or requesting your records; your previous clinic sets its own policy on providing copies. Your first appointment is an initial consultation at $79, and follow-up consultations are $59 to $69. Every fee is published on our fees page before you book.
Do you write to my regular GP?
Only with your consent. Many patients ask us to keep their GP informed so their wider care stays joined up. Others prefer not to, and that choice is respected and documented.
Is my information kept private?
Yes. Your information is handled securely and confidentially, in line with our privacy policy.
