  • GPs and practice managers

    A patient has raised telehealth care with you, or you would like a second clinical opinion without handing the patient over. You keep the patient, and with their consent you hear what happened at each consultation.

  • Allied health

    Physiotherapists, psychologists, exercise physiologists, osteopaths and chiropractors get asked questions that sit outside their scope. You name a clinic, your treatment plan stays yours, and with the client's consent a summary reaches you before their next session with you.

  • Community pharmacists

    A customer asks where to start. You give them a clinic that screens eligibility before any payment and declines where the service is not suitable. LeafLine is not seeking dispensing arrangements from community pharmacies.

  • Support coordinators

    The person you support wants to explore telehealth care and needs a clinic that keeps their existing care team informed. Written consent is captured before anything is shared with anyone.

How it works

How referral works

    You refer

    Use the form below, email admin@leaflineclinic.com.au with the patient's name, phone number and one line of context, or ask the patient to book at leaflineclinic.com.au and name your practice. No clinical records are needed to start.

  2. 02

    We contact the patient within one business day

    The patient-support team calls to complete eligibility screening and, where the patient meets the criteria, books an initial consultation ($79, paid at booking). Patients who do not meet the criteria are told so before any payment.

  3. 03

    An independent clinical assessment

    An AHPRA-registered Clinician reviews the patient's history, medications and previous treatments by video or phone and makes an independent decision. Where the service is not suitable, the consultation is declined and the fee refunded in full.

  4. 04

    A summary comes back to you

    With the patient's written consent, you receive a summary after the initial consultation and after each scheduled review. The patient's existing care team stays in place, and nothing you have in place is replaced by us.

  • Published fees: $79 initial consultation, $69 long review, $59 short review. Patient-pay, no bulk-billing.
  • Decline and refund in full where the Clinician assesses the service is not suitable.

We do not

  • Replace or rewrite the treatment plan the patient has with you.
  • Pay or accept referral fees, in either direction.
  • Charge the referring practice anything.
  • Carry decisions over from another clinic automatically.
  • Share anything with the referrer without the patient's written consent.

Why LeafLine

Nothing changes hands when you refer to LeafLine Clinic, and nothing changes hands when we write back. Our Clinicians are paid a fixed hourly fee with no component tied to what they prescribe, and recommending the clinic earns a practice nothing.

Referring a patient: common questions

Does the patient need a referral to book with LeafLine Clinic?

No. Patients can book directly. A referral means we contact the patient for you and, with the patient's written consent, send a summary back to your practice after the initial consultation and each review.

Do I need to send clinical records?

No. A name, a phone number and one line of context are enough to start. If a medication history would help the Clinician, email it to admin@leaflineclinic.com.au after you submit and the Clinician reads it before the consultation.

What does the referring practice pay?

Nothing. There are no fees to refer and no fees paid for referrals, in either direction. The patient pays the published consultation fees at booking: $79 for the initial consultation, $69 for a long review and $59 for a short review.

Will LeafLine change the treatment plan the patient has with me?

No. Your plan stays yours. The Clinician assesses the patient independently and the summary tells you what was decided. If you would like to discuss it, call (03) 7057 7217 and the team will arrange time with the Clinician.

How does the summary reach me?

By email to the address you give, after the initial consultation and after each review, and only with the patient's written consent. The patient can withdraw that consent at any time.

Is the patient's information kept private?

Yes. Referral details are stored securely in Australia and handled in line with our privacy policy. The form asks for no health information.
